About this activity
Use the QI template below to help you identify priority patients and increase your practice team’s knowledge of recognising and diagnosing dementia.
Goal
Conduct a health assessment for patients aged 75 and over by a target date.
Measures
Use Pen CAT4 to identify the proportion of patients 75 and over who had a health assessment in the previous 12 months. Then see by how much this number has increased by the target date.
Improvement ideas
- Invite clinical staff to complete online training on recognising and diagnosing dementia in the general practice setting.
- Use Pen CAT4 to identify patients eligible for a 75+ health assessment.
- Incorporate cognitive function test in all health assessments (consider the validated screening instruments listed on HealthPathways).
- Create a register of patients with known dementia and those newly diagnosed and use the register to review management.
