NWMPHN Head to Health Data Management System poster

The Australian Government has appointed North Western Melbourne Primary Health Network to run backend systems and support for the national Head to Health program until at least 2026. This poster explains how the system has supported more than 80,000 mental health referrals across Australia to February 2025.
