Urgent Care Clinic and Medicare Urgent Care Clinic providers can print/distribute the following patient experience survey posters and flyers.
The posters/flyers contain two options for patients: to scan a QR code linking directly to the survey, or have the survey emailed to them or sent by SMS.
The surveys are personalised to each clinic across Victoria, and are available in digital (.pdf) and print (.pdf) formats to put up at your clinic:
