The hospital care at home resources package comprises videos and digital information resources designed to increase awareness and understanding of hospital care at home in Victoria.
The creation of these resources was a collaborative effort involving the three Melbourne Health Service Partnerships. Real-life stories of patients and carers were included to provide relatable content that builds awareness and understanding of hospital care at home.
The resources underwent an extensive review process, including input from a Clinical Advisory Group, consumers, and the North Western Melbourne Primary Health Network.
