Hospital care at home – patient information sheet

Hospital care at home is hospital-level care delivered in the comfort of a patient’s own home by teams of health professionals who visit regularly.

Research shows that hospital care at home delivers better health outcomes, fewer complications and a better experience for patients and their carers.

The creation of these resources was a collaborative effort involving the three Melbourne Health Service Partnerships. Real-life stories of patients and carers were included to provide relatable content that builds awareness and understanding of hospital care at home.

The main purposes of these resources are to:

  • Build awareness of hospital care at home as part of the public healthcare system.
  • Promote positive patient experiences and to allay concerns.
  • Provide consistent, easy to understand messages for stakeholders at all levels.
