Victorian Government dementia funding on offer for Brimbank GPs

The Victorian Government is offering grants of $45,000 (excluding GST) to eligible, accredited general practices to implement an innovative model of care to identify and proactively manage patients with dementia and mild cognitive impairment over a 12-month period.

Brimbank has been identified as a local government area (LGA) with a higher prevalence of dementia, and general practices located in this LGA are encouraged to consider applying for a General Practice Innovation Grant.

The General Practice Innovative Grant Program is funded by the Victorian Government in recognition of the valuable role of general practice in improving health and wellbeing and reducing pressure on the health system. The grants program has been codesigned with general practitioners.

Key dates and application details

  • Applications open: 11am, 18 November 2025
  • Applications close : 4pm, 16 December 2025
  • Notification of outcomes: March 2026
  • Program start: April 2026
