
The Royal Melbourne Hospital (RMH) is committed to reducing hospital admissions and emergency department visits.
The RMH Hospital Admission Risk Program (HARP) supports people with chronic conditions – including diabetes – to better manage their condition in the community and reduce avoidable hospital admissions.
The team is made up of six credentialed diabetes nurse educators, a dietitian, and an endocrinologist.
This focused, short-term service can evaluate diabetes management either at the patient’s home or in community-based clinics.
Ongoing communication and teamwork with the patient’s primary care team are key to the program’s success.
Any glucose monitoring results or diabetes care plans will be sent to your clinic for review.
Additionally, GP case conferencing is available for suitable patients to ensure sustainable and cohesive diabetes care is delivered.
