St Vincent’s Hospital Melbourne is progressing to pilot implementation of the Model of Care developed via our 2025 ‘Supported Discharge from Specialist Clinics’ project, with continuing support from Eastern Melbourne Primary Health Network’s ‘CareSynergy’ program.
The Model overcomes the barriers to discharge of clinically appropriate patients from specialist clinics, improves integration between primary and tertiary care, and promotes efficient health service utilisation.
To progress this continuing work, titled ‘Clinics2Community: Supporting Continuing Care’, we are assembling a Primary Care Stakeholder Group to
- provide input into the clinical handover process, and implementation of safety-net services (asynchronous advice and option for re-referral)
- develop resources to aid the continuing care of nominated patient populations following discharge from specialist clinics
- guide evaluation of the GP experience of this model of care.
We are seeking expressions of interest from GPs and practice nurses who are interested and available to attend the following meetings:
- Meeting 1: Thursday 30 April 2026, 9am-11am
- Meeting 2: Thursday 11 June 2026, 9am-11am
- Meeting 3: Tuesday 28 July 2026 (subject to change, may be deferred until August), 9am-11am.
Additional, out of session review of documents will be required, estimated at 4-6 hours over the course of the engagement.
You will be remunerated for attendance at meetings and reading/review of material.
