POLAR data tools: supporting quality improvement, population health planning and research in general practice

North Western Melbourne Primary Health Network (NWMPHN) supports general practices across our region through access to Outcome Health’s POLAR data tools.

These build better understanding of patient populations, identify opportunities for improved care, meet PIP QI requirements, support quality improvement and data-informed decision-making, and support everyday clinical and operational planning.

The transition to POLAR occurred in July 2025. As part of our ongoing support, NWMPHN recently distributed POLAR data quality reports to participating practices. If you have not received yours, please contact us at primarycare@nwmphn.org.au

Use of de-identified general practice data

NWMPHN continues to explore how de-identified and aggregated POLAR data can be used to support population health planning and primary care research across our catchment.

In the interest of transparency, we are formally advising practices of our intent to use aggregated, de-identified data for population health planning and research purposes.

As outlined in the NWMPHN Data Sharing Agreement, de-identified data shared via POLAR contributes to secure, aggregated datasets that supports research and planning activities to improve health outcomes for our communities.

If your practice does not wish for its data to be included in the NWMPHN de‑identified data pool, you may opt out by contacting primarycare@nwmphn.org.au by Wednesday, 25 March 2026.
