Over three Wednesdays in October 2025, members of NWMPHN’s Integration team brought together stakeholders to codesign a new model of care as part of the recently commissioned Health Care Neighbourhood project.
The project supports general practices in the Hume local government area to deliver safe, high quality care through a multidisciplinary team (MDT) approach, thereby improving patient outcomes.
The codesign group included representatives from five organisations commissioned to provide the service: Doy Street Medical Centre, Merrifield City Medical Centre, DPV Health Medical & Dental Centre Broadmeadows, Roxburgh Park Doctors and Holstep Health. Joining them were two community representatives, Dr Karyn Alexander, a GP champion with proven experience in MDT care through a previously commissioned initiative, and our own team members.

Facilitated by management consultants Impact Co, the sessions were highly interactive and energising.
NWMPHN participants gained skills in design-led engagement, co-facilitation, and leading discussions, strengthening internal capability for other collaborative work.
The sessions led to the development of four unique models of care, tailored to the needs and context of each participating general practice.
These will be implemented in the coming weeks, marking a new phase in delivering integrated, multidisciplinary care, reflecting the real-world needs of our region’s patients, practices, and communities.
The Health Care Neighbourhood project will run until June 2027.
