Practice Catalyst

Practice Catalyst is an online self-assessment and improvement tool that helps general practices understand their current strengths and identify opportunities for growth.

Practice Catalyst was designed by Australian health care consultants Rebbeck, and has been customised by North Western Melbourne Primary Health Network (NWMPHN) for local general practice use.

Completion of the Practice Catalyst tool by a practice is voluntary and is free to practices within the NWMPHN catchment.

What does Practice Catalyst measure?

Practice Catalyst is not an audit or compliance tool. It is a starting point for reflection, learning and action.

It is designed for use by practice owners, principals or practice managers.

The tool considers practice maturity over five domains:

  • Practice foundations – operational sustainably and adaptability
  • Practice management – governance, leadership, and administrative efficiency
  • Clinical operations – clinical workflows
  • Patient care – patient-centred approaches, accessibility and safety
  • Community connection – engagement with local health networks and population health initiatives.

Practice Catalyst draws on extensive national and international evidence of what makes general practice effective, sustainable, and patient-centred. Internationally, it reflects models of high-performing primary care that emphasise leadership, teamwork, data-driven improvement, and system integration. Nationally, it aligns with the RACGP Standards for General Practices (5th Edition), PHN quality improvement frameworks, and Australian studies on sustainability, access, equity, and outcomes. What are the benefits for practices to complete Practice Catalyst?

Within two business days of completing the survey, the practice receives a tailored action plan. This provides practical resources to support it in areas across the five domains that may have been identified for improvement.

Responses across these areas present an overall view of strengths and development needs, providing insight into the maturity of practice. Each element assessed within the program results in a score, providing instant insight into areas of operation that are going well and others that might need improvement.

An indication of sustainability is generated across four sub-domains – workforce, financial, infrastructure and access and continuity. It is designed help practices understand operational risks. It is important to note that this indicator is not designed to be a prediction or diagnosis. Instead, it’s a guide to help highlight areas that may need additional support or further exploration.

Following the end of the survey period practices will receive their maturity score benchmarked against the NWMPHN average, as well as an indication of sustainability.

  • Why is NWMPHN providing this tool to practices?

    Once the action plan is received it is ready to use. Another benefit of the Practice Catalyst tool is that it will support targeted engagement by NWMPHN. A primary care facilitator will be in touch to discuss the various types of support available, helping the plan transform into real action.

    The primary care facilitator can help you:

    • interpret your results and identify priority areas
    • connect with training, peer learning, or improvement programs
    • access tools and resources that match your needs and goals.

    Aggregated data will be used to provide insights into practice needs across the region. These can assist with planning general practice engagement and support programs.

  • Data protection and access

    As well as helping individual businesses refine protocols and procedures, the program also shares results with NWMPHN and the administrators of the tool.

    Data is not shared outside these entities.

    Identifiable practice-level data will be accessible only to authorised NWMPHN staff and contracted providers Rebbeck and Semantic Consulting, which administer the Practice Catalyst tool.

    Agreements are in place with contracted providers to ensure secure handling of backend data. Individual practice results will not be disclosed to external parties or government departments. NWMPHN will utilise individual results to work collaboratively with each practice to undertake improvements in identified areas.

    Aggregated data will be used to provide insights into practice needs across the region. These can assist with planning general practice engagement and support programs. De-identified aggregated data may be used in NWMPHN publications to highlight regional trends and inform broader system improvement.

    By completing the assessment, you consent to NWMPHN receiving your responses. These will be stored securely and used in accordance with NWMPHN’s Privacy Policy.

  • How do I access Practice Catalyst?

    Each practice will be sent a secure link via email to complete the tool. Practices must complete the consent section before they can access the questions, which we estimate will take 20 minutes to complete.

  • The bigger picture

    We expect many general practices in the region will opt to use Practice Catalyst. The total data produced will better enable NWMPHN to direct programs, training and resources to where they will matter most.
