Join Gippsland PHN and Prestantia Health in this 3 part learning series to develop knowledge, support and optimising Person-centred and Multidisciplinary Team-based Care.

These education sessions are part of a structured program of three interactive online workshops designed to support general practices at varying stages of their multidisciplinary team (MDT) development journey. The program is tailored to meet practices where they are in terms of understanding, MDT maturity, and readiness for change, with content progressively informed by participant feedback gathered through targeted surveys between sessions.

These workshops will engage GPs, practice managers, nurses, and administrative staff, focusing on enhancing person-centred care and increasing service effectiveness and efficiencies of multidisciplinary team-based care

Workshop 1 – "The Why" Teams Matter Foundations of Higher Performing MDT Care

This workshop will explore the rationale for MDT care (“the Why?”), provide a framework to help participants clarify what high-quality MDT care looks like in practice and for whom, and examine how this approach can address common professional and operational pain points. Learning will be reinforced through case studies, storytelling, and interactive elements such as in-session baseline surveys to assess current state and readiness for change.

  • Addressing practice and professional pain points through MDT approaches

Learning outcomes:

By the end of this session, participants will be able to:

  • Describe the benefits of person-centred, multidisciplinary team-based care in general practice
  • Identify the characteristics of high-performing MDT care and how it applies to their patient population.
  • Recognise common practice and professional challenges and how MDT approaches can help address these.
  • Assess their practice’s current level of readiness and capability to enhance MDT care using reflective tools and surveys.
