Please join this Victorian Primary Health Network collaborative event, for a general overview on the palliative care journey of patients accessing general practice, and how digital health platforms can support optimisation of care. The presenters are Dr Elise Davey, practice principal of Kensington Hill Medical Centre in Leopold Victoria and Dr Paul Molina Chavez, Practice Principal at Wangaratta Medical Centre.
Presenters:
Dr Elise Davey is a GP and practice principal at Kensington Hill Medical Centre, Leopold Victoria, having joined in 2017 and specialised in General Practice. Dr Elise has a strong interest in geriatric care and is a proud provider of care to patients residing in residential aged care facilities. Dr Elise is also a current board member of Hospice Foundation Geelong.
Dr Paul Molina Chavez migrated to Australia over 16 years ago from Ecuador. Dr Paul holds a Clinical Diploma in Palliative Medicine (Royal Australian College of Physicians) and practices as a Rural General Practitioner in Wangaratta. He is also a medical educator for the RACGP and Director of the Medical Degree Year 3 Rural Pathway - University of Melbourne. Dr Paul was awarded the RACGP 2023 Victoria GP of the Year.
