Discover how Palliative Aged Care Outcomes Program (PACOP) tools can support clinicians to identify those residents who are living with end-stage dementia.

Some people in residential care may not being recognised as being at end-stage dementia. It is possible that we are under assessing this group of people.

Join us for our PACOP in Practice online session, where will discuss this group of people we identify as the “Walking Dying”, and how the PACOP tools can help in the identification of their palliative care and end-of-life care needs.
