PACOP in Practice: Better Care, Better Communication - DRT

Discover how the Palliative Aged Care Outcomes Program (PACOP) Deteriorating Resident Tool (DRT) supports timely recognition of resident deterioration and meaningful clinical conversations.

Join us for our PACOP in PRACTICE online session where our Improvement Facilitator Natalie Joseph will explore how the DRT can support clinicians to recognise and respond to signs of resident deterioration, including subtle changes in condition, emerging symptoms, and family concerns.

This session will discuss practical ways to use the DRT to guide clinical decision-making, support timely care planning, identify residents who may be approaching end of life, and ensure appropriate escalation and palliative care pathways are considered when needed.
