COVID Monitor is being introduced into the West Metro COVID Positive Pathway

COVID Monitor is a secure, online portal that allows you to view all of the patients enrolled in the model from your practice in one, clear dashboard. In addition, COVID monitor has self reporting symptom monitoring for patients that will allow flexibility for follow up of patients.

This session is a drop in to ask any questions to the NWMPHN.
