Delivered in partnership by the Victorian, Tasmanian and Adelaide Primary Health Networks, the After Hours in Aged Care webinar series brings together residential aged care staff from across the country to explore practical solutions to current sector challenges.

Join us for this FREE three-part webinar series to support Residential Aged Care Homes (RACHs) to strengthen after-hours care, improve clinical decision-making, and prepare for future models of care.

Session Three: Making Every Record Count: Using Health Records to Improve Care the meet the New Standards

In this session, they will share practical insights into the role of health records, including My Health Record, in supporting continuity of care, information sharing, and better resident outcomes. Attendees will gain practical strategies to strengthen documentation practices, leverage digital health tools effectively, and ensure health records not only support compliance with the new Standards but also contribute to safer, more connected and person-centred care.
