For general practice and primary care teams, National Diabetes Week (12-18 July 2026) is an important reminder of the opportunity to identify people at risk of type 2 diabetes early and connect them with effective prevention supports.
Many patients with risk factors – including overweight or obesity, family history of diabetes, history of gestational diabetes, or cardiovascular risk factors – may benefit from early intervention.
The Life! program, funded by the Victorian Government and delivered by Diabetes Victoria, is a free evidence-based lifestyle program supporting eligible Victorians to reduce their risk of type 2 diabetes, heart disease and stroke.
Delivered by qualified health professionals, Life! provides practical support through group courses and health coaching to help participants build sustainable changes in nutrition, physical activity and overall wellbeing.
GPs, practice nurses and primary care health professionals are strongly encouraged to discuss diabetes risk with patients and refer those eligible to the Life! program as part of routine preventive care.
