
A HealthPathways Melbourne page has been created to reflect the Australian Government’s changes to the MBS Chronic Disease Management Framework, starting on 1 July, 2025.
Called ‘Chronic Condition Management Items’, the page covers the introduction of the General Practice Chronic Condition Management Plan (GPCCMP), including the relevant new item numbers. It features a streamlined view of the changes and includes direct links to the corresponding MBS Online resources.
The changes simplify the referral process and improve access for patients. Here’s what you need to know:
- The GPCCMP items replace the previous General Practice Management Plan and Team Care Arrangement items.
- The MBS fee for the GPCCMP and its review are now equal, encouraging reviews and continued care.
- Allied health referrals have been streamlined, with the previous requirement for consultation with two other providers removed.
- Providers can now issue a referral letter rather than using a specific form. The letter does not need to specify the allied health provider or the number of sessions. This allows greater flexibility and empowers patients to choose what works best for them.
Key points for eligibility and access
Eligibility criteria remain unchanged. Patients with one or more chronic medical conditions, described as lasting or likely to last at least six months, or terminal, remain eligible.
There is no definitive list of conditions. Clinical judgement continues to guide eligibility.
Patients enrolled in MyMedicare must access GPCCMPs through their registered practice. For those not enrolled, access must be through their usual GP. See this HealthPathways Melbourne page for more information.
