About this activity
Use the QI template below to better record influenza vaccinations for patients with chronic obstructive pulmonary disease (COPD) and meet the requirements for the PIP QI Incentive.
Goal
Increase the percentage of patients with COPD who have had an influenza vaccination recorded in the past 15 months.
Measures
POLAR measure: Percentage of patients with COPD who have had an influenza vaccination recorded in the past 15 months.
Improvement ideas
- Conduct a targeted blitz to ask all patients with COPD who attend the practice within the activity period; or
- Use POLAR filters to identify all active patients with COPD at the beginning of flu season to invite for a flu shot; or
- Use PIP QI Report inPOLAR to pull a list of patients with COPD who have not had a flu vaccination in the past 15 months and send an SMS or email reminder to encourage them to book in for a flu shot.
