Registering your patients with MyMedicare – using Pen CAT to identify priority populations

If your practice is already registered with MyMedicare, you may want to start looking into how you will identify patients eligible for registration.

Not sure how to do this? Pen CS have provided some helpful resources to get you started.

Pen CS CAT4 recipes detail how to target cohorts relevant to MyMedicare, including:

  • Identifying patients at risk of hospitalisation
  • Identifying patients who have had or are due for review of their CDM Plan
  • Identifying patients in a Residential Aged Care Facility (RACF)
