It provides practical resources and guidance to support planned, preventative care and enhance coordination with multidisciplinary teams to improve patient outcomes.
Practices can utilise the toolkit to streamline their workflows, access a range of helpful resources, and implement continuous QI activities, ultimately fostering a patient-centred approach in aged care settings.
This QI toolkit has been developed by PHNs working together nationally through the PHN Cooperative, the National Improvement Network Collaborative and the National MyMedicare PHN Implementation Program.
