New clinical program to support chronic disease management

Western Health’s Nephrology and Endocrinology Units is offering a new clinical program that aims to prevent or delay progression of chronic disease in our community, focussing on chronic kidney disease, cardiovascular disease, and diabetes care.

It is for general practices located in the local government areas of Brimbank, Hobson’s Bay, Hume, Maribyrnong, Melton, Moonee Valley, Wyndham, Moorabool.

The clinical program uses Future Health Today, an innovative clinical decision support technology that was co-designed with GPs, practices nurses, practice managers and consumers, to be intuitive, useful, and fit within general practice workflows.

There has been excellent feedback from staff participating in the previous pilot program.

“I don’t think it adds much time at all just to have a look. Again it’s just a prompt. It’s easy. It tells you there and then what you need to do which – that’s why I actually am a fan of it. I think it’s easy to use.”  – Practice nurse.

“…I love it in such a way that basically whatever recommendation, I can easily copy them and clipboard and copy and paste into the file.” And “not only that you’re looking after the welfare of the patient, but also, you’re looking after the welfare and the business side of the GPs…”  – General practitioner.

Western Health clinicians will provide clinical education and support, access to a chronic kidney disease nurse practitioner (a new service) and increased diabetes educator support as part of the program.
