Joint projects demonstrate success

The Collaborative – a partnership between North Western Melbourne Primary Health Network (PHN), cohealth, Merri Community Health Services and The Royal Melbourne Hospital – held its fourth annual forum last week.

Dr Douglas Travis, Urologist and Chair of The Travis Review 2014-15: an enquiry into Victorian hospital capacity and innovation, gave a key note address speaking about the outcomes of the review and opportunities for The Collaborative to innovate.

The forum was also an opportunity for the partnership to celebrate its success. Presentations about joint projects on Advance Care Planning, Back pain and eHealth demonstrate, in a very practical way, the health benefits of working together.

Our new website http://collaborative.org.au/ – which was launched at the forum – provides details about these projects and other joint initiatives on Chronic Kidney Disease and Diabetes, and outlines the aims of The Collaborative.

CEOs Ms Lyn Morgain – cohealth; A/Prof Christopher Carter – North Western Melbourne PHN; Mr Nigel Fidgeon – Merri Community Health Services; and Dr Gareth Goodier – The Royal Melbourne Hospital; signed a Collaborative Framework (2016 – 2020) that formalises the group’s ongoing commitment to better health in our shared communities and outlines how success will be measured over the next five years.
