On 14 November, we’re celebrating World Diabetes Day. On this day (and every day), help your patients reduce their risk of developing diabetes.

The Life! program is a free healthy lifestyle program that helps people reduce their risk of type 2 diabetes, heart disease and stroke. Diabetes Victoria coordinates the Life! program and is funded by the Victorian Government.

The program offers engaging, informative, and practical evidence-based education sessions for your eligible patients to help them adopt healthy behaviours and lead a more active lifestyle. Our health professionals will work with your patients to help them learn more about:

  • type 2 diabetes, heart disease and stroke
  • healthy eating
  • physical activity
  • stress management and wellbeing
  • goal setting and managing lapses.

It is also offered in Chinese, Vietnamese, Arabic and plain English.

Exciting program updates:

The Life! program have launched Life! Online, a digital tool that your patients can use from their desktop, mobile or tablet to continue learning between sessions. They will have access to videos, quizzes, readings, and weekly tips.

The Life! program also offers a Financial Incentive Agreement for GP clinics, providing practices an opportunity to receive a financial incentive of up to $45 plus GST, for screening and referring eligible patients to the program.
