Thank you for allowing North Western Melbourne Primary Health Network (NWMPHN) to film you for its sex worker health project.

Where may your image be be used?

By completing this form you give permission to NWMPHN to use the footage to produce educational videos to be used in sex worker health training sessions for primary care providers.

These sessions will be closed and only participants will see the videos. The videos will not be published publicly online or on social media, and will not be used for advertising or promotional purposes.

The videos may be used in these education sessions until 30 July 2023.

Any proposed changes to the consent you provide today (for example: changes to the context in which the videos will be used, target audience or time limits on their use) will be discussed with you first and be subject to you renewing your consent.
