Join us for an information session on NWMPHN’s transition from Pen CS (CAT4) to Outcome Health (POLAR) as our clinical audit tool provider. This session will provide an overview of the transition process and include time for questions and discussion.
This Q&A session will answer all your questions and address any concerns.
Our transition will ensure efficient data collection without disrupting data-sharing capabilities or incentive payments for practices.
What’s changing and why:
Learn about the upcoming switch to POLAR, an innovative and widely used clinical and population health data analytic system, provided by Melbourne-based not-for-profit Outcome Health.
Two sessions available. Bring along any questions and concerns for us to address and clarify, so you feel confident and informed throughout the transition. These might include how to update data-sharing agreements, ensure smooth installation of POLAR software, and maintain smooth incentive compliance.
