Magali De Castro - Nurse Educator and Primary Care Leader, Practice Nurse Central
Magali is a Nurse Educator and Primary Care Leader with more than 20 years' experience supporting general practice teams across Australia.
She is the founder of Practice Nurse Central and has worked extensively with practices, Primary Health Networks and healthcare organisations to improve preventive care, chronic disease management and nurse-led models of care.
Her previous roles include Chronic Disease Service Integration and Practice Support Coordinator for Melbourne's Bayside region and Clinical Director at HotDoc. She has been a presenter and guest speaker for organisations including the Australian Primary Health Care Nurses Association (APNA), the Victorian Department of Health, Exercise and Sports Science Australia (ESSA) and the Cancer Council among others.
Magali is passionate about helping practices maximise opportunities for prevention, chronic disease care and patient engagement through practical, and sustainable approaches that support both patients and healthcare teams.
Lauren Hwang - Practice Nurse, Nurse Practitioner Candidate and APNA Clinical Facilitator
Lauren is a highly skilled Registered Nurse and current Nurse Practitioner Candidate. She holds a Bachelor of Nursing alongside postgraduate certificates in Primary Care Nursing, Women's Health, and Sports Medicine. Lauren is an accredited nurse immunizer and cervical screening provider. As a Nurse Practitioner Candidate, she practices at an advanced practice level, delivering more in-depth health assessments to her patients. Lauren is also an APNA Clinical Facilitator, hosting student nurses for clinical placements and mentoring peers transitioning into primary health care. Additionally, Lauren is a valued member of the General Practice Expert Advisory Group at NWMPHN.
Learning outcomes:
- Describe the purpose and use of the main Health Assessments in General Practice and identify key Medicare (MBS) requirements relevant to these.
- Differentiate the roles and responsibilities of Nurses and GPs in the planning, delivery and documentation of health assessments.
- Identify practical approaches to finding eligible patients and strategies that can improve the efficiency and consistency of health assessments in practice.
