HELP Community of Practice - April

Join us for an informal chat with other people who are using the Healthy End of Life (HELP) app or referring it to patients, clients and carers.

This is an opportunity to share ideas, discuss what is working along with any challenges you might be experiencing. This will be an open facilitated discussion with a supportive group.

The HELP app is an easy to use tool that activates family and friends to provide practical support for someone at the end of life. We know that network-centred care improves end of life outcomes and integrating it into service delivery can take as little as 5 minutes.

Session is delivered via Zoom. Access link will be provided after registering.
