Join one of our engagement focus groups to develop and build on existing high quality support in the North Western Melbourne PHN region.

North Western Melbourne PHN has commissioned Siggins Miller to consult with primary care providers to understand how to better engage with the sector. The aim of this consultation is to develop the highest quality person-centered and integrated care approach with important input from primary care practitioners. This strategy and the future model for engagement will be informed by consultation findings as well as international evidence, health reforms, the Patient Centered Medical Home (PCMH) clinical model.

North Western Melbourne PHN invites general practitioners, allied health, pharmacists, dentists, aboriginal health workers and primary care nurses to come along to one of the consultation evenings.

Reimbursement for attendance
Register now online, join the focus group and contribute to the strategy. Listed participants will be reimbursed (flat rate of $108.75 inc. GST) and required to complete pre-reading prior to session.
