North Western Melbourne Primary Health Network (NWMPHN) seeks to commission one suitable organisation to provide a highly mobile two-person team to work with selected general practices in Hume. It will play a key role in delivering multidisciplinary care to patients with complex needs.

The team will comprise a clinical care coordinator and a social worker. It will meet every week with clinicians and patients at four Hume practices. It will assist staff in assessing patient need, delivering care, supporting patient self-management, and coordinating access to other allied health and support services in the local area.

This is a collaborative project, which will see the selected organisation work closely with NWMPHN and participating general practices to codesign and implement a unique and effective multidisciplinary care model.

Suitable organisations might come from the community health, allied health or adjacent sectors.

Funding of $769,962.94 (ex. GST) across two years (FY25-26 and FY26-27) will be provided to the successful applicant.
