Overview

North Western Melbourne Primary Health Network commissioned Aboriginal Community Elder Services Inc. (ACES) to provide social and emotional wellbeing supports to residents of its Brunswick East residential aged care home.

The program aims to improve the psychological health and wellbeing of residents by:

  • delivering a range of individual and group supports that are responsive to the specific social and emotional wellbeing needs of residents
  • enhancing existing resources for Elders to better support their mental health
  • providing linkages to other services and supports as required.

The service focuses on increasing psychological wellbeing through a range of activities including internal and external cultural activities. Service providers also work to develop partnerships with relevant social and emotional wellbeing services for counselling and other supports.

Outcomes

A key outcome for this program is to improve access to culturally safe social, emotional and wellbeing support for First Nations people in residential aged care.
