Use the QI template below to better record blood pressure (BP) for patients with diabetes and meet the requirements for the PIP QI Incentive.
Goal
Increase the percentage of patients with diabetes who have had a BP recorded in the past 6 months.
Measures
Pen CAT4 measure: Percentage of active patients with diabetes who have had a BP recorded in the past 6 months.
Improvement ideas
- Use Pen CAT4 to identify patients with diabetes who have not had a BP recorded in the past 6 months.
- Invite them in for diabetes care (BP check and other care or tests as needed).
- Incorporate use of resources to enhance care.
