The 2026 World Diabetes Day theme, “Early action on diabetes,” recognises a critical reality: many people living with diabetes remain undiagnosed, while millions more are at high risk of developing type 2 diabetes.

According to the World Health Organization, more than 40 per cent of people living with diabetes are unaware they have the condition, highlighting the importance of identifying risk and acting early

Every day, GPs and practice nurses see patients with risk factors. These patients may not need treatment today, but may benefit from support to prevent chronic disease in the future.

The Life! program provides eligible Victorians with free evidence-based support to reduce their risk of type 2 diabetes, cardiovascular disease and stroke.

Supported by qualified health professionals, participants are guided to make sustainable lifestyle changes before chronic disease develops. For busy practice teams, a referral to Life! provides a practical way to connect at-risk patients with structured support to reduce their risk of developing chronic disease.

From raising awareness of risk factors to connecting patients with preventive programs, primary care plays a vital role in helping people act before diabetes develops.

This November, make the most important diabetes conversation the one that happens before diabetes develops.
