Supporting diabetes prevention with the Life! program

World Diabetes Day (14 November 2025) is a timely reminder of the growing impact of type 2 diabetes across our communities.

This year’s theme, ‘Know more and do more for diabetes at work’, highlights the importance of supportive environments, including workplaces and health care settings in improving outcomes for people at risk.

Primary care plays a vital role in prevention. The Life! program, funded by the Victorian Government, is a free, evidence-based initiative that supports eligible adults to reduce their risk of type 2 diabetes, heart disease and stroke. Delivered by qualified health professionals, Life! offers group courses and telephone health coaching focused on sustainable lifestyle changes.

We encourage practices to refer eligible patients to the Life! program and promote it as part of your preventive care efforts. Resources and referral pathways are available to make integration easy.
