North Western Melbourne Primary Health Network’s 2024-25 Annual Report is now available.
This year’s report highlights our continued progress in transforming health care across the region. It showcases the programs and projects we commissioned to meet the evolving needs of our communities, and celebrates the work of our dedicated primary care providers.
It also reflects our commitment to supporting them through education, practice support and strategic partnerships. There are several in-depth features, exploring how selected projects affected the sector and the communities it serves.
The report also marks a decade since NWMPHN was founded, and includes some entertaining reflections and thoughts from some of our longest-serving team members.
