Activating Equity is the refreshed Access and Equity Framework for NWMPHN.
NWMPHN is committed to addressing inequities to improve the health and wellbeing outcomes of community members, especially those at greatest risk of poor health.
This refreshed framework contains perspectives, tools and resources, making it a flexible guide for stakeholders and commissioned providers.
