Our Community Participation Plan outlines our goals for community participation at North Western Melbourne Primary Health Network and the steps we are taking to achieve them.
This year we are reviewing the Plan and we need your help – tell us what you think of our current Community Participation Plan by completing a 10 minute anonymous survey. The feedback we receive will inform the development of the 2023 – 2026 Community Participation Plan.
Once you’ve completed the survey, you can choose to enter a draw to win one of four $50 visa gift cards (your name will not be linked to your survey responses).
