Can you help us paint a picture of the health care system in Melbourne’s northern and western suburbs? We’d love to hear from you by Friday, 9 June 2023.
North Western Melbourne Primary Health Network (NWMPHN) is working with the Royal Melbourne Hospital, Peter MacCallum Cancer Centre, the Royal Women’s Hospital, Western Health, Mercy Health and the Royal Children’s Hospital on a project to better understand health needs in what is perhaps the most densely populated and diverse region of Australia.
We want to hear from people living or working in Melbourne’s northern or western suburbs, the CBD, Macedon Ranges or Bacchus Marsh about the health needs across the region and their experience of accessing health care services.
By gathering this information, we and our partners will be able to see where new services are needed, or where existing services need to work more closely together. We can then take real steps to improve the quality of care and service for everyone in the region – making us all healthier and happier.
How to take part
Click this link to complete the survey.
In our survey we want to hear from a very wide range of people. We want to know about how you access health care, what difficulties you have in getting treatments, what’s working and what’s not.
You can complete the survey yourself or on behalf of a child or someone else you are caring for.
The survey takes only about 20 minutes to complete, and you don’t need to have any information at hand before starting.
At the end of the survey, if you wish, you can enter the draw to win one of 5 $100 EFTPOS gift cards. To enter, you will be asked to provide a contact email address or phone number. This will not be used to identify you – all your responses are anonymous.
The survey closes at 5pm on Friday, 9 June 2023.
This survey is available in English only and we understand that surveys are not suitable for everyone. We’re addressing this by directly engaging various groups of people. To learn more about the project visit this page.
Are you a health care provider?
We have a different survey for you to complete. Find it here and learn more about the project here.
