North Western Melbourne Primary Health Network’s Model of Care promotes a person-centred approach to mental health, suicide and alcohol and other drug treatment and care, supporting services which are responsive to community needs and expectations. Services commissioned under this Model of Care deliver targeted and appropriate services to a wide range of community and cultural groups at numerous locations through our region. Services are presented according to age group, target population, location, treatment type and level of vulnerability and need to assist people to find the best service for their individual needs.

North Richmond Community Health

The Care Navigation pilot supports participants and service providers to address barriers to accessing the supports and services needed to improve health outcomes for people with AOD and co-occurring physical and mental health, including risk of suicide, chronic disease and other complex needs.

The service is delivered in North Richmond with assertive outreach in the City of Yarra and the City of Melbourne.

Target Population: People who use alcohol and/or drugs and have a physical and/or mental health issue who require assistance to navigate the service system.

Youth Projects

The Substance Misuse Treatment Program is a primary health service that provides health care and support to improve the physical, mental and social wellbeing of individuals who are homeless or at risk of homelessness and experiencing comorbid alcohol and other drug and mental health concerns.
