Our 2021 Community Health Needs Survey is now closed.

Thank you for your input. Your responses, and those of other people in our region, will be used to help us understand what health services are needed.

About the survey

One of our roles as a Primary Health Network (PHN) is to plan and fund health services that are needed in our region.

Every three years we complete a Health Needs Assessment. This involves looking at health data and talking with our local communities, health care organisations and other stakeholders to better understand the needs of people in our region.

Your answers help us to plan better health care in our region

The responses from this survey will be analysed along with:

  • responses from another survey that will be sent to service providers and other organisations
  • health data (for example, data from the Australian Bureau of Statistics, hospitals and general practices)
  • consultations with community groups, service providers and peak organisations.

Our Reference Group (an internal working group) and our Community and Clinical Councils will review the data and discuss priority areas and how we might address these.

The report of all these findings becomes our Health Needs Assessment. We will use this to work with our communities and service providers to design and improve health services and activities.

Our People Bank is a register of people who would like to help us to improve the health of people in the north, west and central Melbourne area. Everyone in our community is welcome to join.

As a People Bank member, you will receive emails about opportunities to help us with our work. People Bank offers payment for some activities. Interpreters will be made available for you if needed.

North Western Melbourne Primary Health Network is a not-for-profit organisation funded by the Australia Government.

Our role is to help improve the health of people living in central, northern and western Melbourne. We do this by planning and funding health services that communities need.
