As part of Western Health’s Prostate Cancer Survivorship project, this interactive session will update attendees on the shared model of care for prostate cancer provide participants with practical approaches to management of common issues in prostate cancer survivors. 

This highly interactive education session will allow attendees to hear from experts in prostate cancer survivorship about common issues faced in primary care. Following an overview of our shared care model for prostate cancer, participants will break in to small groups to discuss the common issues and frequently asked questions of our patients.

Learning outcomes:

Attend this session and you will increase your knowledge and skill sets in order to:

  • Describe the importance of communication on survivorship issues between general practice and Western Health
  • Identify common issues among prostate cancer survivors (sexual well being and relationships; mental health; and lifestyle including diet, physical activity and alcohol)
  • Demonstrate confidence in discussing common areas of concern regarding prostate cancer with patients in a general practice setting
  • Utilise systematic preventative health management and use of recalls and reminders to support shared care
  • Implement the Western Health Shared Model of Care for Prostate Cancer
