The activity aims to inform GPs and practice staff in the West of Melbourne on the effective use of new therapies and technology for management of Type 1 and Type 2 Diabetes patients in primary health care setting.
This session will be facilitated by the Diabetes and Endocrine department at Sunshine Hospital and will be lead by Dr Dev Kevat (Endocrinologist, FRACP) and Deepa Ariarajah (CDE, RN)
This session will provide information on:
- new treatments (oral and injectable) for Type 2 Diabetes
- information on the changes to NDSS, new technology such as flash glucose meter, continuous glucose monitor (CGM) available for patients with diabetes
- Discuss cycle of care and software available for PHC team to promote patient engagement
- Post-discharge management of patients with type-2 diabetes
- Interactive discussions of case scenarios
Learning outcomes:
By the end of the session all participants will be able to:
- Motivate patients to commence injectable therapies, including insulin
- Demonstrate knowledge of benefits and risks of second and third line oral and injectable therapies
- Demonstrate an understanding of new blood glucose monitoring technologies for Type 1 & Type 2 diabetes
- Provide care to their patients that can align with the diabetes cycle of care
- Describe the principles of best practice management of patients with Type 2 Diabetes (post-discharge from specialist clinics)
