North Western Melbourne Primary Health Network (PHN) is looking to commission the design and implementation of service interventions that improve rates of healthy birth weight (>2,500g) in Melton South and Wyndham. 

The market briefing aims to: 

  • inform potential providers about the intent and scope of the commissioned activity. 

  • gather feedback from potential providers about the proposed design of the commissioned activity. This will inform further development of the final specification and Request for Tender, currently scheduled for release in October. 

The briefing will cover: 

  • Background for the activity 

  • Discussion of key elements of service interventions. These include but are not limited to:  

  • Support families through a response-based approach 

  • Identify causes and social determinants of low birth weight outcomes  

  • Generate collective impact through partnerships for collaborative, evidence-based and locally targeted responses based on community needs and preferences 

  • Ensure services are accessible and culturally safe for all families and children  

  • Deliver improved low birth weight outcomes / improvements in key indicators relevant to the service response  

  • Support innovation and adaption to emergent realities for children and families in Melton South and Wyndham through developmental evaluation  

  • Implement evidence-based outcome measurement that demonstrates progress towards improving healthy birth weight rates 

  • Improve equity of outcomes for vulnerable children and families  

  • Contribute to service system integration and build service models that are sustainable post-funding 


Who should attend? 

Health and community service providers in Melbourne’s west who provide, or are interested in providing, services to children and families in the first thousand days, particularly the period between conception and birth.
