Advance Care Planning One Day Workshop

Health Professionals are invited to register for the Advance Care Planning one day workshop

This one day workshop focuses on the key elements of advance care planning, including ACP in 3-steps:
A - Appoint an Agent.  C - Chat and Communicate.  P - Put it on Paper.  

This workshop is suitable for those health professionals who want to know: 

• What advance care planning is about 

• How to introduce the subject to other people 

• How to assist people with advance care planning decisions 

• How to complete the advance care planning documents 

Learning outcomes:

At the end of this session you will be able to:
