Identify patients at an increased risk of COVID-19 infection and possible subsequent long COVID. Resolve to raise these matters within a certain timeframe.
Measures
Use Pen CAT4 to identify patients considered at increased risk of COVID-19 infection and severe illness, and those at higher risk of developing long COVID.
Improvement ideas
- Set reminders for ‘higher risk’ patients for winter planning.
- Use Pen CAT4 to pull a list of patients who are at a higher risk of COVID-19 infection and subsequent long COVID.
- Use pre-existing templates for at-risk patients enabling them to quickly access the best treatments available if infected.
