Use the QI template below to increase your practice teams knowledge of where to find advance care planning documentation, and how to identify which patients need an advance care plan. It also promotes knowledge of local palliative care teams, and how to help patients remain in their own homes for as long as possible.

Increase staff rating their knowledge of palliative care services and supports as ‘high’.

Staff survey rating knowledge of palliative care services and supports as low, medium or high.

Improvement ideas

Ensure relevant practice members know where to locate advance care planning documentation.
Review your advance care planning documentation processes to identify further improvement opportunities.
Ensure all relevant practice team members know how to identify the appropriate care to support patients, carers and families.
Request a free education visit from a local palliative care provider to help the practice team understand the services available and referral requirements.
