WSS Eligibility

Available to all ages in our region.

TPSS Eligibility

  1. Adults with a diagnosed mental illness and clients under the age of 18 with a provisional diagnosis;
  2. People who may not be able to afford fee based services;
  3. Priority population groups including: people at risk of homelessness; Aboriginal and Torres Strait Islander peoples; people seeking asylum and refugees; people transitioning out of the justice/ corrections systems; and people identifying as LGBTIQ+.

ISS Eligibility

  1. Have a diagnosed mental illness as described under the DSM V – whose severity and complexity requires significant clinical care coordination;
  2. Have a history of hospital admissions (mental health) OR has received an Episode of Care from Area Mental Health Services;
  3. Not be currently engaged in Area Mental Health Services;
  4. Be able to be managed in a primary health care setting;
  5. Be voluntarily engaged with written patient consent obtained to participate in the ISS;
  6. Be willing to engage in a recovery focused service that is person-centred and goal driven;
  7. Be a current patient of a psychiatrist, or requires linkage with a psychiatrist;
  8. There is no other appropriate service.

(NOTE: The ISS cannot be considered as a substitute for Area Mental Health case management)

PSS Eligibility

Psychosocial Support Services are for people who:

  • would benefit from time-limited, non-clinical psychosocial support
  • are ineligible for the NDIS
  • are not currently receiving services through the National Psychosocial Support Transition (the extension of the former Partners in Recovery (PIR), Support for Day-to-Day Living in the Community (D2DL) and Personal Helpers and Mentors Service (PHaMS) services
  • do not access State Government funded community mental health support services (Early Interventions Psychosocial Support Response (EIPSR) program)

The services will provide much needed support for people with severe and complex mental illness who are not eligible for assistance through the NDIS.

SPPS Eligibility

  1. Adults recently released from hospital or emergency services following an attempted suicide or incident of self-harm;
  2. People who are at increased risk of suicide, but do not warrant referral to an emergency service;
  3. People can access this service even if they have accessed Medicare Better Access in the current calendar year or Targeted Psychological Services.

DDS Eligibility

Available to all ages. No referral needed.

AOD Eligibility

Services are for individuals and families seeking treatment and support related to harms being experienced with the use of alcohol, prescription medications and/or other substances.
