Chronic disease

The Chronic Disease Support Program is designed for health professionals who are new to the field of chronic disease self-management.

In the current status of the current COVID-19 health emergency, this session is scheduled to progress as planned.  

Should the advice from the Chief Health Officer escalate mitigations to minimise public gatherings, we will reconsider.

We will keep you informed of any changes as they become known to us.


The program explores a range of topics including assessing risk factors, readiness for change in health behaviours, while working with both the client and health professionals to develop a chronic disease self- management plan.

This nationally accredited program is driven by a desire to create better health for clients, reduce hospital admissions, better patient compliance and the provision of care by those best qualified to provide advice. On successful completion of the programs, students should have the ability to support practitioners to work collaboratively with people managing chronic disease.

The one-day workshop session involves several interactive group activities covering various aspects of chronic disease self-management, developing a chronic disease self-management plan and group case studies.

Course delivery and assessment for the Chronic Disease Support Program includes a one-day face-to-face workshop, in-class activities assessed by facilitator, and an online assessment completed via BenchmarqueIQ.

The Chronic Disease Support Program is a nationally accredited program and is fully funded by the federal Department of Health under the Chronic Disease Project.

Learning outcomes:

By the end of this session you will be able to:

  • Discuss risk factors of chronic disease and high risk population groups
  • Assess and support an individual with a chronic disease
  • Apply management strategies, client centered education and related support in the area of chronic disease
  • Assist patients to develop self-management plans and identify areas for health behaivour change
 

COVID-19 update to everyone registering for our training and events: 

The situation is continuing to evolve quickly. At present there are a small number of confirmed cases in Victoria and all our events will go ahead as planned.  

Nonetheless, it is critical that we all remember the principles of infection control and take extra steps to guard against the spread of COVID-19: 

  • WASH your hands often with soap and running water, for at least 20 seconds. DRY with paper towel or hand dryer. 

  • TRY not to touch your eyes, nose or mouth. 

  • COVER your nose and mouth with a tissue when you cough or sneeze. If you don’t have a tissue cough or sneeze into your upper sleeve or elbow.  

  • HAND SANITISER is available in all our meeting rooms. Please use this. 

  • SHAKING HANDS is optional! 

  • MAINTAIN at least 1 metre distance between yourself and anyone who is coughing or sneezing. 

  • DON’T ATTEND if you are sick or experiencing COVID-19 symptoms (fever, breathing difficulties such as breathlessness, cough, sore throat, fatigue or tiredness).
